Many of our office and case managers have prior experience in the senior care field, providing them with valuable hands-on knowledge. Home Care Makes a Difference

As we advance in age, we recognize that our elderly have earned a badge of pride in their fight against father time, and all that comes with the aging process.

Home care is an important option for the elderly and their family. Home care provides added support not only for the patient, but a relief for the family who has endured the many responsibilities that aging encompasses. Providing those “second set of hands,” with a health care professional, relieves both the patient and his loved ones of the difficulties and responsibilities of daily activities.

Gray hair is a crown of life, and growing old should be a testament of achievement and success, not hardship and adversity. Growing old with dignity is our goal, and home care is the vehicle to achieve that dream.

Once you have discussed the patient’s needs with our professional intake personnel, we will begin the process of customizing the specific services, days and hours the patient requires.

At the start of services, one of our registered nurses will arrange to come to the patient’s home, coordinate with his physician or Medicaid Plan, and begin to develop a unique Plan of Care specific to the patient’s individual needs. We always respect their individuality and wishes.

As the patient’s advocate, HOME Agency not only helps reduce the need for hospitalization or institutional care but allows the patient to enjoy the comforts of home while receiving one-on-one care.

And of course, once service begins, HOME Agency is available 24 hours/day, 7 days/week to respond to any questions, concerns or emergencies.
