Pending Medicaid
For patients with pending Medicaid application, HOME can offer the option of private pay. Our HOME intake team will guide you through the process until services are provided under Medicaid.
Medicaid Application
For families who require assistance obtaining community Medicaid for their aging loved one, HOME offers expert application services that are proven to lead to acceptance the first time around. HOME hopes to become community’s go-to application center. HOME is also proud to assist families in setting up Chronic Care Medicaid, which is designed for those entering a nursing home. HOME can also help families set up a pool trust for their loved one, should such measures be necessary. By making both services accessible to families of any means, HOME strives to ensure that every elderly member of the community is covered.
Enrollment Services
Once our clients are approved for Medicaid, HOME serves as the liaison between the patient and receiving homecare services. Our intake registered nurse will guide the patient and family through necessary evaluations. These enrollment services are provided completely free of charge to all patients, and our clients are typically enrolled within 2-3 months of beginning the Medicaid application process.
Geriatric Management & Nurse/ Social Work Services
HOME boasts a well-rounded roster of social workers and registered nurses that provide a myriad of in-home services to our patients. From pre-pouring medications and setting up doctor’s appointments to managing mail and shopping needs. Social workers assisting in applying for SNAP, HEAP and entitlement benefits. The social services we provide ensure that patients and families are as free from stress as possible.

The Home and Community Based Services Medicaid Waiver for Individuals with Traumatic Brain Injury (HCBS/TBI):
The TBI waiver uses Medicaid funding to provide supports and services to assist individuals with a traumatic brain injury (TBI) toward successful inclusion in the community. Waiver participants may choose to move into the community from a nursing facility. Others may choose to participate in the waiver to prevent unnecessary institutionalization. HCBS/TBI waiver services are available to supplement informal supports; broad array of local, State and federally funded services, as well as Medicaid State Plan services to assure the health and welfare of the individual in the community.
The Nursing Home Transition and Diversion (NHTD):
NHTD Medicaid waiver is a Home and Community–Based Service (HCBS) program. The NHTD waiver provides services to assist individuals with disabilities, and seniors, toward successful inclusion in the community. Waiver participants may come from a nursing facility or other institution (transition) or choose to participate in the waiver to prevent institutionalization (diversion). Waiver services are provided when informal supports, local, state, and other federally funded are not sufficient to assure the health and welfare of the individual in the community.
Home and Community Support Services (HCSS)
HCSS are the combination of personal care services (Activities of Daily Living (ADLs) and Instrumental Activities of Daily Living (IADLs)) with oversight and/or supervision as a discrete service. Services are provided primarily in a participant’s home. HCSS is provided to a waiver participant who requires assistance with personal care services tasks services and whose health and welfare in the community is at risk because of oversight and/or supervision of the participant is required even when no personal care task is being performed.
